Note to Applicant and Parent/ Guardian: The Ossoli Club May Queen contest is a fundraiser for school and community projects in the City of Corbin. Every contestant is required to raise a minimum of $500 to participate in the final court. Donations are nonrefundable. All donations are to the Ossoli Club Foundation which is a legally IRS-registered 503C entity allowing for tax deductions to the fullest extent. It is recommended that persons wishing to use their donation as a tax deduction pay online at ossoliclubofcorbin.org or write a check payable to Ossoli Club Foundation in order to have record of their donation. Only single donations greater than $250 will be issued a receipt. Your signatures indicate your desire to participate as a May Day Queen contestant and your permission as parent/guardian for the contestant to do so.
NOTE: THE OSSOLI CLUB WILL SELECT 21 CANDIDATES FROM THE APPLICATIONS RECEIVED BY 12MN ON 2/25/22 VIA EMAIL to mayday@ossoliclubofcorbin.com (NO EXCEPTIONS WILL BE MADE)
